This includes questions about your general health, any oral diseases and treatments you have had in the past and any medications (antibiotics or other medications) you are taking. Medications may interfere with the products used in our dental treatments. That is why it is important to provide us with a complete list of medications. Please note that this information will remain confidential and that only the people linked to your file will have access to it.<\/p><p>Please fill out the form below and send it to us before your first appointment, or have it with you when you arrive at the office.
