At GK Dental Centre in Brossard, our mission is to provide accessible quality dental services in a calm and warm environment. Our reception team will take the time to create your file and guide you through our range of services.
New patient questionnaire
To speed up the process of creating your file and to get you in touch with your care team, we ask all new patients to fill out a first visit form. This includes questions about your general health, any oral diseases and treatments you have had in the past and any medications (antibiotics or other medications) you are taking. Medications may interfere with the products used in our dental treatments. That is why it is important to provide us with a complete list of medications. Please note that this information will remain confidential and that only the people linked to your file will have access to it.
Please fill out the form below and send it to us before your first appointment, or have it with you when you arrive at the office. This will help us create your file and give our dentists all the necessary information to take care of your oral health!
File transfer
You can also send us your dental records. Legally, your records are the property of your previous dental clinic. However, your former dentist is obligated to provide you with a paper or electronic copy if you request one. You may be charged a reproduction fee. Once you have your records, all you have to do is send them to us or bring them with you to your appointment with your dentist in Brossard.
